•  2022-2023 Immunization  Requirements 

    All parents of children entering Pre-k or Kindergarten, as well as grades 1,3,5,7,9 & 11. 
    ​Please be advised that your children will need their well child exam/physical before, or within 30 days of starting t
    he 2022-2023 school year. Please plan accordingly with their primary care physician so this can be accomplished. ​

    Students entering Kindergarten will need their 2nd MMR and varicella, as well as their 4th polio and 5th DTAP. Children entering 6th grade will need their TDap vaccine.

    Those entering 7th, 8th, 9th, and 10th grade will need a Meningitis vaccine, and those entering their senior year will need their​ 2nd Meningitis vaccine​ if their first dose was received prior to age 16. 
    The physicals and the vaccines are state-mandated and need to be obtained in a timely fashion.

    2022-2023  Health Examination Requirements

    New York State law requires that all new entrants (whether Pre-K or K), 1st, 3rd, 5th, 7th, 9th and 11th graders, and all students entering a school district for the first time have a health examination by a New York State licensed provider. This examination must be provided to the nurses office within 30 days of school. 

     During this school year, the following screenings will be required or completed at school:
     
    Vision:
         - Distance acuity for all newly entering students and students in Kindergarten, and Grades 1,  3, 5, 7 and 11.
         - Near Vision acuity and color perception screening for all newly entering students. 
     
    Hearing:
         - Hearing screening for all newly entering students and students in Kindergarten, Grades 1, 3, 5, 7 and 11.
     
    Scoliosis:
         - Scoliosis (spinal curvature) Screening for girls in grades 5 and 7 and boys in grade 9. 
     
    Health Appraisals:
         - A physical examination including Body Mass Index and Weight Status Category Information is required for all newly entering students and students in Pre-K or Kindergarten, Grades 1,3,5,7,9, and 11.  Physical forms can be viewed and/or printed from our Forms section so that you can take a copy to your Primary Physician.
       
    Dental Certificates:  
          - A Dental Certificate is requested but not required for all newly entering students and students in Pre-K/Kindergarten, Grades 1,3,5,7,9 and 11.
